This completed form will be used by the LSINW 2012 Recruitment and Selection committee to select organizations that will present at the conference.

These fees include attendance for one presenter and one additional organization or board representativePayment is due by June 11, 2012.

Presenting Organization Fees:
If your organization is selected to present you will receive, with the notification, an invoice for presenting organization fees.

Private companies: $1,000.00 USD
Public companies: $1,500.00 USD
Non-profit organizations:
$750.00 USD
Academic Institutions:
$750.00 USD
Emerging companies (Yr. 1 & 2):
$500.00 USD
